Truckload Accessorial Charges Effective 01/01/08

Mileages are to be computed from the most current version of PC Miler (practical option)
unless otherwise specified.

All rates are stated in U.S. currency

Stops in Transit: $75 for the first stop; $100 for the second stop; $125 for the  
third; $200 for each stop thereafter.  (When rates are stated in dollars per vehicle used or in cents per hundred pounds, circuitous miles are $1.60 per mile)

Load/ Unload Charge: $150 minimum for driver assist
 
Two-Man Driver Teams: Additional .15 per loaded mile, when requested            
New York Surcharge  Additional $350 for any destination or stop(s) (for partial loading
Zips: 100-104, 106-107,  or unloading) in these zips.
110-119

Maximum Cargo Liability: Carrier's maximum liability for any loss or damage to cargo shall not under any circumstances exceed Two Hundred Thousand Dollars ($200,000) for any single truckload shipment unless a greater degree of liability has been specifically assumed in writing by an authorized representative of carrier prior to tender of a shipment.

Hazardous Material Service: .15 per mile hazmat service charge, or a $75 minimum.
Charge $150 per trailer wash out if required.

Reconsignment or Diversion: $150, subject to stop in transit rules

Returned, Undelivered $1.50 per mile for all miles from origin and returning back to the
Shipments origin point, subject to a $550 minimum  

Truck Ordered, Not Used: $200

Dunnage Disposal: $250 charge assessed if carrier is responsible for disposal of materials used in securing the shipment for transport.

Impractical Operations: $1.50 per mile exceeding shortest route.

Detention: - with power $60 per hour or $15 per 15-minute interval after 2 hours free at each origin and/or destination. Carrier is required to receive written approval from customer before detention charges are applicable. Maximum of $800 per 24-hour period.

Detention: without power $35 per 24 hour period, minimum of 2 days free time must be allowed at origin and/or destination before charges will apply for any equipment. Any charges must be agreed to in writing by both shipper and Innovative before billing.

In-Bond Charge: $100 per in-bond shipment

Ferry Charges: All applicable round trip ferry charges

C.O.D. Shipments: $100 per C.O.D. shipment

Truck Ordered Not Used: $200

Layover Cost: $600 per day
